Save time by printing out the forms you'll need for your office visit and completing them at home.  Remember to bring them with you to your appointment!  Even if you complete all forms at home, please arrive 15 minutes early for your appointment so we can xerox your insurance information and check your vital signs.

what to bring

1. medications!  We need to know exactly what you're taking so we can take good care of you!  Please bring either a medication list (including all medications with doses and how often you take them) or all your pill bottles in a bag and we'll sort them out

2. any office forms that you completed online

3. any old records that you have

arriving for your appointment

We recognize that your time is valuable, and we strive very hard to be as punctual as possible, but we need your help in doing so!  We request that all patients (even established ones) arrive 15-20 minutes early.  This allows us to update your address/insurance information, check your vital signs, and arrange for any preliminary testing (such as doing a urinalysis, rapid strep, EKG, etc.) before you see your provider.  Arriving early allows you to spend more time face-to-face with your provider rather than filling out forms! 

In fairness to our other patients, we regret that patients who arrive more than 10 minutes late may have to reschedule their appointments.  Patients who cancel within 24 hrs or fail to show for an appointment may be billed $25.

Finally, we ask in advance for your patience and understanding on the days when we are running late.  In the vast majority of cases, we are late because of an emergency or because a patient unexpectedly needs extra time and attention on that day.  Please know that we would extend the same extra attention to you, though we hope you stay healthy enough that you don't need it!

what to expect at your appointment

We have several types of appointments, including physical/well-woman exam, urgent care appointments, follow-up appointments, and pre-op appointments.  A short description of each one is below so you can know what to expect. 

  • An annual physical or well-woman exams  We use this time to review all of your medications, perform a complete exam, make sure you are current on recommended preventive care, such as routine labs, mammograms, prostate exams, bone density exams, colonoscopies, vaccinations, and more.  Because your insurance will only cover one preventive exam a year, we recommend making separate appointments for any other concerns rather than trying to address these at your physical.  Many insurance companies insist that it be 365 days since your last physical.  If you aren't sure when your last physical was, you can check with your insurance or contact our office. 
  • Urgent care visits  These are short visits focused on an acute problem, such as a sore throat, recent injury, urinary tract infection, etc.  We always have same-day urgent care appointments so you can be seen and taken care of quickly, not in 2-4 weeks!  For us to offer same-day availability, however, we are not able to address multiple problems at an urgent-care appointment. 
  • Follow-up appointments These visits are designed to keep track of a chronic condition, such as high blood pressure, high cholesterol, diabetes, heart disease, thyroid problems, and others.  We monitor your responses to medications and lifestyle changes and ensure that you are current on recommended interventions for that condition. 
  • Pre-op appointments  These visits are generally requested by a surgeon to ensure that you are safe to undergo a planned procedure.  Pre-op appointments are not the same as a annual physical, because we particularly focus on your cardiovascular and respiratory fitness, your ability to undergo general anesthesia, and your risk of peri-operative complications, such as infection and blood clots.  If your surgeon has given you any papers, please bring these with you to your appointment.
